What the certificate needs to say
Requirements differ by country and by the organisation receiving the translation, so your template should match the markets you serve. Two common cases:
United Kingdom
There is no single legal definition of a certified translation in the UK. The Chartered Institute of Linguists advises that the translation comes with a certificate stating that it is a true and accurate translation of the original document, the date of the translation, and the name and contact details of the translator or of a representative of the translation company. Organisations such as UK Visas and Immigration may ask for more, such as a signature, so check the guidance for the specific application.
United States (USCIS)
For immigration filings, the regulation says that any document in a foreign language submitted to USCIS "shall be accompanied by a full English language translation which the translator has certified as complete and accurate, and by the translator's certification that he or she is competent to translate from the foreign language into English."
